In recent years, silicone breast implants have far outpaced the popularity of saline when it comes to breast augmentation. Reasons that patients (and sometimes even their surgeons) express for this preference include concerns about surface rippling, wrinkling, and other contour irregularities that can occur over time with saline. There are also concerns that saline implants seem too full through the upper pole compared to silicone, or that implant edges may be visible, especially in women with little existing breast tissue for coverage. These days, the general consensus seems to be that silicone implants respond and feel more like natural breast tissue compared to saline implants.

Developed by Dr. Robert Hamas, a Dallas-based plastic surgeon, the Ideal® Implant design is currently undergoing an extensive national clinical trial and is awaiting FDA approval pending the trial’s successful completion. What sets the Ideal® Implant apart from existing saline implant models is its unique system of valves and chambers that allow saline to flow freely within the standard implant shell, lessening the likelihood and severity of contouring irregularities.
The makers of the Ideal® Implant state that their goal is to combine the best qualities of silicone and saline implants:
- Silicone is prized for its natural-looking look and feel
- Saline is valued for its lower cost and “natural” solution to breast augmentation
For women who want saline but also want a very organic feel and appearance, the Ideal® Implant promises to deliver both.
The implant looks just like a normal saline implant, but it has a gentler contour and lower profile so that it creates a more gently sloping look on the chest wall. Inside the implant are a series of separate shells connected by valves that allow the saline solution to move responsively while minimizing the likelihood of wrinkling or rippling in the outermost shell.
Is Ideal® Right For You?
At this time, the Ideal® Implants are not yet available because they’re still in the trial phase and have yet to receive final FDA approval. Until this happens, patients who are interested in this option may want to check with their surgeons on the possible future status of these new implants. Those plastic surgeons involved in the trial, which began in 2010, have given positive feedback so far, but time will tell whether the FDA agrees with these independent opinions.
In the meantime, women still have a choice between saline implants, silicone implants or the newer highly-cohesive silicone gel (“gummy bear”) implants. While much lip service is given to the perceived inferiority of saline when compared to silicone, the truth is that many women love their saline implants and are thrilled with the breast augmentation results they’ve been able to achieve with saline.
